Additionally, we may collect entity’s tax ID number, or social security number (from individuals), and business name and location from Service Providers and Brokers.
More specifically, Employers must provide the following information when creating an Employer Account: legal business name, entity type, EIN or Tax ID, contact person’s first name, middle name and last name, designation/position/title of the contact person, ownership status of business (private, public, non-profit), industry type, total number of employees, whether the Employer provides health insurance to its Employees or not, entity’s main address, fax number, and email address.
Moreover, when creating an Employee list on the Website, the Employer must provide the following information: Employee ID, Employee’s email address, first name, middle name, last name, DOB, Employee’s payment contribution towards Vouchers in percentage, Employee’s work department, job type (full-time, part-time, seasonal, contractor, etc).
2. Usage Data
We may also collect information on how the Website is accessed and used ("Usage Data"). This Usage Data may include information such as your computer’s Internet Protocol address (e.g. IP address), browser type, browser version, unique device identifiers, diagnostic data, the pages of our Website that you visit, the time and date of your visit, and the time spent on those pages.
3. Location Data
We may use and store information about your location if you give us permission to do so ("Location Data"). We use this data to provide features of our Website, to improve and customize our Website.
You can enable or disable location services when you use our Website at any time by way of your device settings.
